Sculptra zones explained — 3 anonymized cases: temples, sunken face, cheeks

In one paragraph

Sculptra works differently in each anatomical zone. The same 2-vial course can transform sunken temples and barely register on sagging cheeks — or vice versa. We see this in the consult room every week. This post walks through three composite cases at Waleerat (details anonymized to protect patient privacy) — temples in early 40s, sunken mid-face after weight loss in late 30s, lower-face cheeks at 52 — to show what 'the right dose for your face' actually means in practice. Each case explains the presenting concern, the dose mapped, the timeline, and the result at month 6. Reading time: 9 minutes.

01Why anatomy decides dose

Sculptra's PLLA microparticles need to land in the deficit AND reach a critical mass to trigger meaningful collagen rebuild. Different facial zones have different volumes of deficit and different surrounding tissue depth — so 'the right dose' is geography, not arithmetic.

A 1-vial course mapped to one specific temple is sub-therapeutic if spread across both cheeks. The same vial deployed correctly at one temple can be transformative.

The three cases below are composites of recurring patient patterns at Waleerat — details adjusted to remove identifying information. Photos shown are general case examples, not these specific patients.

The clinical decisions described are the typical reasoning we apply in the consult room.

02Case 1 — Temples (early 40s)

Presenting: a patient in her early 40s, mostly bothered by sunken temples visible in side-angle photos. Mid-face and cheeks were age-appropriate but the temple hollows photographed as 'tired' under harsh light.

She had previously tried 2 cc of HA filler at another clinic — improvement lasted ~10 months and she didn't want to keep refilling.

Plan: 1.5 vials of Sculptra mapped specifically to the bilateral temple zone — 0.75 vial per side, deep periosteal placement. Single session, ฿37,500 total.

The temple is one of the highest-visible-return zones for Sculptra because the deficit is well-defined, the surrounding tissue is thin, and the patient sees the change in their own mirror within 12 weeks.

  • Week 1: diluent swelling settled in 72 hours
  • Week 4: subtle change — patient unsure if it was working
  • Month 3: clear improvement, patient sent us a 'thank you' message
  • Month 6: full result, no further sessions needed
  • Total cost: ฿37,500 · expected duration 2.5 years

03Case 2 — Sunken mid-face (late 30s)

Presenting: a patient in her late 30s, lost 12 kg over the past year and reported 'looking gaunt.' Mid-face fat-pad atrophy was the main complaint — upper cheeks flattened, light no longer caught the cheekbone, and friends had started commenting that she looked tired. She wanted natural recovery, not 'filled' look.

Plan: 2 vials of Sculptra across the bilateral upper cheek + mid-face zone — 1 vial per side, supraperiosteal placement with cannula. 2 sessions spaced 6 weeks apart. ฿50,000 total.

Mid-face is the most common reason patients book Sculptra at Waleerat (we estimate ~60% of cases) because the volume loss is what photographs most obviously aging in this demographic.

  • Week 1: mild swelling, no visible change
  • Week 6: second session, mid-face already 'softer'
  • Month 3: cheekbone light-catch returning, friends noticed
  • Month 6: photographed result — return to pre-weight-loss face
  • Total cost: ฿50,000 · expected duration 2.5 years

04Case 3 — Sagging cheeks (early 50s)

Presenting: a patient in her early 50s with lower-face sag — cheeks drooping toward the jowls, jawline definition lost. She'd considered thread lift but wanted to start with a less aggressive option.

Important: lower-face sag is partly volume loss AND partly skin laxity. Sculptra addresses the first, not the second.

Plan: 2.5 vials over 3 sessions, mapped across mid-face + jawline + lower-cheek transition zone. ฿62,500 total.

We told the patient upfront that the result would be 'restored structure + better light-catch' but NOT a thread-lift level of repositioning — for that we'd need to add threads at month 6 if she wanted more. This honest framing is critical: Sculptra under-promised is better than Sculptra over-promised.

She also committed to the 5-5-5 massage protocol after each session.

  • Month 1-3: subtle improvement in cheek fullness
  • Month 6: jawline definition returning, jowl shadow softer
  • Patient declined adding threads — happy with Sculptra alone
  • Maintenance plan: 1 vial at year 2 to extend the result
  • Total cost: ฿62,500 · expected duration 2-2.5 years

05Pattern across the three

Three patterns we see repeatedly across hundreds of Sculptra cases:

First: visible-return-per-vial is highest in the TEMPLES, mid-tier in the MID-FACE, and lowest in the LOWER-FACE. This is because deficit volume vs Sculptra dose ratio is most favourable for temples.

Second: patient satisfaction at month 6 correlates more with EXPECTATIONS SET AT CONSULT than with vial count. Patients told 'this will rebuild structure' are happier than patients told 'this will lift your face.' Third: the 5-5-5 massage protocol matters more than patients expect — cases that skipped it had visibly worse results.

Sculptra is not a one-size-fits-all course. The free physician consult is where the right plan for your specific anatomy gets mapped — not in a brochure.

06Common questions

Why are these cases composites and not specific patients?+
Patient privacy is non-negotiable at a medical clinic. Even anonymized case writeups carry risk if specific details (age + anatomy + outcome + photo) combine to identify a real person. The cases described are typical patterns we see weekly across hundreds of Sculptra patients — details adjusted, identifying information removed, photographic examples generic. The clinical reasoning is real; the specific patients are not.
Will I get the same result as Case 2 (the mid-face one)?+
We don't promise so — and any clinic that does is overpromising. Results vary by age, skin condition, fat distribution, lifestyle, and adherence to the 5-5-5 massage protocol. The cases show what's ACHIEVABLE in typical scenarios. Your free physician consult maps what's likely for YOUR face based on your specific anatomy + history.
Can I see real before/after photos of Waleerat patients?+
Yes — in the consult room only, with patient consent for in-clinic display. We don't publish real patient B/A photos on the public website except for patients who have signed explicit consent (currently we have one such case visible). This protects your privacy too — when YOU become a patient, your photos stay private unless you specifically consent.
Can I do all three zones at once?+
We can map a course that addresses temples + mid-face + cheeks across 3 sessions, typically 2.5-3 vials total. But the order matters. We usually start where the patient's CHIEF complaint is — the zone that bothers them most when they look in the mirror. The other zones get layered in sessions 2-3 based on response. Treating everything at session 1 makes it harder to attribute response to specific zones.
